Request Assistance

  1. Contact your nearest NDAD office or call (800) 532-NDAD to request an application for NDAD's Direct Financial Assistance Program.
    • On the map below, click the city office or your county of residence for the application. 
    • Virtual Appointments Available: Current and potential clients can call or email the office in their region to schedule a virtual meeting via Microsoft Teams.

  2. Completely fill out and sign the application.

  3. Print and mail, email, or fax your application to your NDAD office (address listed on page 3 of the application), or drop it off at one of our offices. Add the words "Attn: Client Services" to the mailing envelope, or in the email subject or fax message line.
NDAD's direct financial assistance application also is available in a larger font size. Contact a client services representative to request it.
