  • 2025 Federal Income Tax and/or 2025 SSA Yearly Benefit Letter/2026 COLA Award Letter
  • Child Support Income Ledger, Unemployment documentation or any other income verification
  • Medical insurance cards (Medicare, Medicaid, Prescription Card)

(DO NOT BRING BANK STATEMENTS.)
