NDAD introduces in-town medical travel program TAP to Fargo, 3 neighboring cities
NDAD, with help from a grant provided by Swanson Cares, currently offers the Transportation Assistance Program (TAP) for in-town medical travel in Fargo, West Fargo, Moorhead and Dilworth to residents of those four communities.

Delaying or foregoing medical care because transportation is unaffordable may lead to declining health or death.

TAP covers transportation costs to Fargo, West Fargo, Moorhead and Dilworth facilities for services such as wellness and prenatal check-ups, behavioral health treatment, dental cleanings and eye exams, among others. Qualified applicants receive gas cards or city bus fare as funding is available.

To qualify:
* You must receive services by a local, licensed medical professional.
* Your appointment must be medically necessary (restrictions apply).
* You must already receive assistance from an eligible state/federal program.

Download and complete the application and submit it to NDAD's office at 21 University Drive N., Fargo, ND 58102, email fargo@ndad.org, or fax (701) 281-8216.

NDAD. It's for you.
